Сбор информации проводился путём анкетирования персонала отделений позитронной эмиссионной томографии в период с 2012 по 2017 г. Всего обследовано 19 отделений позитронной эмиссионной томографии из 12 регионов Российской Федерации, девять из которых оснащены собственными отделениями производства радионуклидов и радиофармпрепаратов. Почти все отделения позитронной эмиссионной томографии в России укомплектованы современными позитронными эмиссионными томографами, совмещенными с рентгеновскими компьютерными томографами, которые предназначены для проведения двух процедур в рамках одного исследования. В каждом отделении собиралась информация о видах исследований, применяемых радиофармпрепаратах и активностях, вводимых стандартному пациенту (масса тела 70 ± 5 кг), а также параметрах протоколов компьютерного томографического сканирования и дозах облучения пациентов. Эффективные дозы, полученные пациентами за одно совмещенное позитронное эмиссионное компьютерное томографическое исследование, определялись как сумма доз внутреннего облучения от введенного радиофармпрепарата и внешнего рентгеновского облучения при компьютерном томографическом сканировании. Согласно полученным данным, в России проводятся исследования всего тела с 18F-фтордезоксиглюкозой, 18F-холин, 11С-холин, 68Ga-PSMA, 68Ga-DOTA-TATE, 68Ga-DOTA-NOC, исследования головного мозга с 18F-ФДГ, 11С-метионин, 18F-холин, 18F-тирозин, исследования миокарда с 13N-аммоний. Дозы облучения пациентов от совмещенных исследований лежат в диапазоне 3–40 мЗв. Наибольшие дозы получают пациенты при исследовании всего тела – около 17 мЗв, при исследовании головного мозга – 3,4– 4,8 мЗв, при исследовании миокарда – 2,7 мЗв. При этом компьютерное томографическое сканирование вносит от 65% до 95% в дозу облучения пациента при исследовании всего тела и 20–30% при исследовании головного мозга. При дополнительных многофазных компьютерных томографических сканированиях с введением контрастного вещества доза может увеличиваться до 15 мЗв при исследовании головного мозга, до 25–30 мЗв при исследовании всего тела и до 35–40 мЗв при исследовании миокарда. Для оптимизации проведения позитронных эмиссионных исследований, сравнения результатов, полученных в разных отделениях позитронной эмиссионной томографии, и повышения диагностической ценности данного метода в России необходима стандартизация используемых протоколов сбора и обработки данных. Оптимизация радиационной защиты пациентов возможна за счёт использования низкодозового режима компьютерного томографического сканирования вместо диагностического, создания специальных протоколов для отдельных групп пациентов, корректного применения системы автоматической модуляции силы тока (tube current modulation) и современных методов реконструкции изображения; обучения медицинского персонала техническим приёмам снижения дозы и знаниям в области радиационной безопасности.</p></abstract><trans-abstract xml:lang="en"><p>This study presents an overview of the most common positron emission tomography examinations in Russia, as well as the acquisition protocols and patient doses. The data collection was performed in 2012–2017 in 19 positron emission tomography departments in 12 regions of the Russian Federation by questioning the staff. The majority of the Russian positron emission tomography departments were equipped by modern positron emission tomography scanners combined with computed tomography. In each investigated department, data on all types of positron emission tomography examinations, radiopharmaceuticals, administered activities used for standard patient (body mass 70±5 kg) and parameters of computed tomography protocols was collected. The effective doses of patients from combined positron emission computed tomography examinations were estimated as a sum of the dose from the internal exposure (injected radiopharmaceutical) and the external exposure (computed tomography scan). Whole body positron emission tomography examinations in Russia were commonly performed with 18F-fluorodeoxyglucose (18F-FDG), 18F-choline, 11С-choline, 68GaPSMA, 68Ga-DOTA-TATE, 68Ga-DOTA-NOC, brain examinations – 18F-FDG, 11С-metionine, 18F-choline, 18F-tyrosine, myocardial perfusion – 13N-ammonie.</p><p>The highest patient effective doses (about 17 mSv) were observed for whole-body positron emission computed tomography examinations; for brain examinations – 3,4 – 4,8 mSv; for myocardial perfusion – 2,8 mSv. The computed tomography scan contributes up to 65 – 95% to the total patient effective dose for whole body examinations; 20 – 30% for head examinations. For the multiphase computed tomography scan effective doses may be increased to: 15 mSv for head examinations, 25 – 30 mSv for whole body examinations and 35 – 40 mSv for myocardial examinations. A standardization of acquisition and processing protocols is necessary for optimization of positron emission tomography examinations in Russia and for the intercomparison of results obtained in different positron emission tomography departments.
