В основе методики лежат модели риска Публикации 103 МКРЗ, коэффициенты пожизненного радиационного риска для российской популяции и собственные результаты оценки типичных доз облучения пациентов, подвергающихся различным медицинским исследованиям в Российской Федерации. Для 30 исследований, определяющих около 80% коллективной дозы населения Российской Федерации от медицинского облучения, радиационный риск был рассчитан с использованием «золотого стандарта», а именно органных доз и соответствующих половозрастных коэффициентов риска для российской популяции. Для остальных медицинских исследований (за исключением маммографии) для оценки радиационного риска использовали значения коэффициентов риска, рассчитанных на 1 мЗв эффективной дозы, представляющей собой ее усредненное значение для 4 выбранных анатомических областей тела: голова, шея, грудная клетка и брюшная полость — таз. Предполагается, что в рамках такого подхода погрешность оценки риска возрастет по отношению к погрешности оценки риска, оцененной для вышеупомянутых 30 исследований, не более чем на ±30%. Показано, что значения пожизненного риска смерти с учётом вреда от снижения качества жизни по причине онкологического заболевания и наследственных эффектов варьируют в зависимости от пола и возраста пациента различным образом в зависимости от того, какие органы подвергаются облучению во время медицинского исследования. Оценки риска для некоторых исследований и значений возраста и пола пациента, рассчитанные с использованием «золотого стандарта», могут отличаться от полученных с помощью эффективной дозы и номинальных коэффициентов риска МКРЗ, усредненных по возрасту и полу, до порядка величины. Продемонстрированы различия в оценках риска у пациентов, связанные с использованием коэффициентов риска для российской и композитной популяции.</p></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to develop a methodology for assessing radiation risk for patients undergoing medical examinations in the Russian Federation. The methodology is based on the risk model of the ICRP Publication 103, the coefficients of lifetime radiation risk for the Russian population and the results of evaluating the doses of patients in the Russian Federation. For thirty examinations that define about 80% of the collective dose of the population of the Russian Federation from medical exposure, the radiation risk was calculated using «gold standard», — the organ doses and the corresponding sex/age risk coefficients for the Russian population. For other examinations (with the exception of mammography) the values of the risk coefficients normalized on 1 mSv of effective dose, which is the averaged value for four selected anatomical areas of the body: head, neck, chest and abdominal cavity — pelvis, were used. It is assumed that for such examinations the error of risk assessment will increase relative to the error of risk assessment for the aforementioned 30 examinations not more than 30%.
